If at any time you cannot be reached at the work phone number I have on file, I must be informed and supplied with a number at which a parent can be reached.

No child will be released to anyone other than those listed on the Child Pick-Up form. At the time of pick-up I will request a Photo ID to be shown to me.

The State of Illinois requires that authorities must be contacted should a child be left at a childcare facility after business hours.  The provider will attempt to reach a parent by phone and if unsuccessful, call the people listed on the child pick-up list.  If no contact and arrangements have been made by an hour of contracted time of pick-up, the police and DCFS will be notified and the child will be turned over to these authorities.
